The application form must contain (seeAPPLICATION FORM.pdf):

- Name and surname of parent / guardian
- Name and surname of the child (also of the personal companion, if he or she will be with the child)
- Child's date of birth
- Allergies of the child
- Medicines the child is taking

After receiving the application formfollowed by an individual conversation with parents/guardians. 

An individual conversation with parents/guardians serves to get to know each other before being involved, and to create an individual plan. In our conversation, we address behavioral peculiarities, answer possible questions, drug therapy and dietary peculiarities.
